Job Description
Salary: $100 to $130 per visit
Job Type: Full-Time or per Diem (Monday-Friday or Wednesday-Sunday)
Per Diem – Make your own schedule
Location of choice: Brooklyn, Queens, Bronx, Manhattan and Nassau County
frf Timing: 9-5, or 10-6
Responsibilities:
1. Conducts initial assessments and reassessments with creation of Plan of Care for the member.
2. Ensures safe delivery of member care in accordance with policies and professional nursing standards.
3. Performs comprehensive, accurate physical assessments of member, and records the data on our agency forms properly.
4. Records thorough and accurate health histories and initial nursing assessments.
5. Identifies member health problems accurately and comprehensively.
6. Communicates member’s conditions, responses and needs to medical practitioners and other professionals as needed to promote management of the member’s health needs.
7. Participates in obtaining physician orders for services per agency policy.
8. Plans and performs interventions to ensure that needs are met in an appropriate and timely manner.
9. Formulates and revises nursing care plans to consistently reflect the needs of the member in managing health problems.
10. Implements the nursing care plan with appropriate explanation of care and teaching to members, our staff members, and members’ significant others as needed.
11. Teaches and demonstrates, insofar as possible, health care techniques or procedures to member/ member’s family.
12. Creates a positive environment conducive to team building and progressive nursing practice.
13. Participates in performance improvement activities in compliance with policy and professional standards.
14. Continuously assesses and evaluates the quality of services delivered in order to ensure the highest possible level of care and customer satisfaction.
15. Demonstrates assertive and creative problem-solving ability by identifying problems affecting member satisfaction, service quality and efficiency and makes recommendations for improvement.
16. Formulates paraprofessional care plans. Orients and instructs the paraprofessional to the care plan, reporting requirements, and agency policy.
17. Supervises the paraprofessional. Performs evaluation, teaching, and records such activities as required by the agency, according to policy.
18. Informs the physician and other personnel of changes in member condition.
19. Documents assessments, visits, communications, conferences, care plans and discharge planning and summaries by agency standards.
20. Completes admission and discharge paperwork in a timely fashion.
21. Completes all required documents for work time, time off, etc. in a timely manner. 22. Participates in professional development.
Qualifications:
1. Licensed and currently registered to practice professional nursing with the New York State Education Department.
2. At least one year of home care experience within the past 3 years is preferred.
3. Demonstrates ability to assess and plan for the needs of clients and families with knowledge of medications, treatments and therapies.
4. Fluency in Spanish preferred.
5. Ability to communicate effectively orally and in writing.
